What Does Your Health Benefit Plan Include?

Members of the Laborers of Western Canada Locals 92 and 180 are notified of their Plan when they are provided with the new eligible package which includes the benefit summary. Members are eligible for the benefit plan based on the contribution rate that their employer remits. Members are always enrolled in the highest possible option.

Forms & Documents

These forms are for submitting paper claims, applying for disability benefits, freezing your hours, changing your banking information, updating an address, adding/changing a dependent, and assigning a beneficiary for your life insurance, among others.

You can submit your forms via email or fax, but they must have a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.

Accessing Your Benefits

As a member of the Labourers of Western Canada Locals 92 or 180, or as a member of the Dock and Shipyard Workers of Local 1611, you can access your benefits by submitting claims for medical expenses incurred.

You can submit health claims electronically by logging into the Ellement App; however, some providers have the option to direct bill.
